Professional Membership – Clinic Team
The lead Doctor or Nurse in each clinic will provide CHIVA the names of all clinic team members (included designated members of their linked adult clinic team) to be provided with CHIVA professional membership. 3 of whom will be nominated to have full voting rights (should include the lead consultant and nurse). Clinic membership is life long. Clinics can opt out of Clinic Team membership at any point.
Cost: £50 per centre, a one-off payment to cover life-long membership
